Repeat Prescriptions

Many patients are on regular medication which can be requested without seeing your doctor. When you are on regular medication you will be given a computer printout of the list of medication. Please select which items you require by ticking each item and then returning your prescription form to the surgery.

You can: -

bulletHand it in at reception (or leave it in the basket provided)
bulletPop it through our letterbox
bulletFax it to us on 01372 379941 for Fetcham
bulletFax it to us on 01372 362099 for Leatherhead
bulletSend it in the post
bulletSend it in the post, provide a SAE and we will post it back

Your prescription will be ready after two working days (or if you give it to us on Friday it will be ready by Tuesday).

If you do not have a repeat prescription request form please write your name, address, doctor and items required on a piece of paper.

Please indicate if you would like the prescription to be sent to a chemist of your choice.

The chemists used by our patients are:-

Fetcham - Cannon Court, Fetcham & Bookham Pharmacies
Leatherhead – Day Lewis , Buckley Pharmacy & Boots.

If you request your prescription through your chemist it takes us the same amount of time to process.

Common Questions and Answers—Repeat Prescriptions

Q. When should I put in my repeat request?
A. When you have one week’s supply of tablets left. If you are going away you can put in your requests earlier than usual — just put a note on it so say why you are requesting your prescriptions early.

Q Why don’t you take repeat prescriptions over the telephone?
A. The surgery has over 6000 patients and we issue prescriptions for 6300 items every month. If every patient telephones us, we would not be able to get ay other work done and the telephone would be constantly
engaged. Once you have used the computer print out you will see how easy it is.

Q. I have lost my computer print out. What should I do?
A. No problem! Just drop a piece of paper into the surgery with your name, address, your doctor’s name and the medication you need. When you collect your prescription there will be a new computer list for you to keep.

Q. Will you post my repeat prescription to me?
A. Yes of course. We will need a stamped addressed envelope.
If you do not give us a SAE, we will keep your prescription at the surgery for you to collect.

Q. Can you send my prescription to any chemist/
A. No. The only chemists that collect are the ones that have a collection
