Official FormsPrivate Medical Insurances/Forms etc.Non-NHS Examinations: Medical examinations for special purposes, e.g.: Fitness to drive, Pre-employment, insurance medicals etc., can be arranged by appointment. The fee recommended by the British Medical Association is charged for these examinations, which are not available under the NHS. Insurance claim forms, Passport application forms, etc.There are a variety of forms, which require a doctor’s signature. The doctors will complete these for you but please allow 3 working days. The fee recommended by the British Medical Association will be payable on completion by the doctor and can be collected from the surgery reception. Travel InjectionsNot all travel injections are covered by the NHS. They may be classified as Non-NHS Services and therefore chargeable. Please make sure you check what vaccinations you will require with the surgery before you travel. Ideally you should contact us two months before you travel. Please note that we do not accept credit cards.
