THE MOLEBRIDGE PRACTICE
The practice has two surgery sites, one in Fetcham, the other in North Leatherhead. You will be registered with a particular doctor but you can request to see any doctor in the practice. This practice operates as a partnership and the group consists of 6 doctors. General InformationRegistering With The PracticeWe welcome new patients to our practice. When you register with one of our doctors you will be required to put your new details onto your medical card that was issued to you by the health authority when you registered with your previous GP. If you have mislaid this then you can fill in a New Patient Registration form, which we have in the practice, and at that time you will be asked to return and make an appointment with a practice nurse for a health check within one month unless you need repeat medication. Then an appointment should be made earlier to confirm medication. Then book with the doctor to have medication authorised. This involves an examination and questions will be asked about your current and past health in order that we may identify any preventable medical problems. You will also be required to provide a urine specimen. You may see anyone of our doctors anytime, not just the doctor you are registered with. Travel InjectionsNot all travel injections are covered by the NHS. They may be classified as Non-NHS Services and therefore chargeable. Please make sure you check what vaccinations you will require with the surgery before you travel. Ideally you should contact us two months before you travel. Please note that we do not accept credit cards.
